Global MedTech News | FDA Histotripsy Rules, MRI-to-3D AI & HOYA Divestiture (Oct 5–11, 2026)

Oct 5–11 MedTech news: FDA histotripsy and home-sampling rules, MRI-to-3D AI clearance, remote stroke robotics, Zenflow funding and HOYA divestiture.

The MedTech week of October 5–11, 2026 was shaped by U.S. device-classification rules, AI-assisted imaging, and cross-border commercialization. FDA codified Class II special controls for non-thermal focused-ultrasound tissue ablation and home-collected STI testing, while MRI2CT announced clearance of software that reconstructs 3D lumbar bone anatomy from routine MRI.

Business activity was equally notable: Cook Medical backed Zenflow's $52 million financing and signed an exclusive distribution agreement, while Japan's HOYA agreed to sell its ophthalmic surgical-instrument subsidiary MID Labs. European cartilage-implant commercialization and a Brazilian embolization-device authorization rounded out a geographically diverse week.

1. FDA Codifies Class II Special Controls for Non-Thermal Focused-Ultrasound Tissue Ablation

Publication/effective date: October 6, 2026

FDA added 21 CFR 878.4405 for focused-ultrasound systems that mechanically ablate soft tissue without relying on thermal injury. The category, associated with histotripsy, is Class II with special controls. Importantly, this October 2026 rule codifies a De Novo decision issued to HistoSonics for its Edison System in October 2023; it is not a new product authorization this week.

The controls address clinical and animal evidence, acoustic characterization, off-target tissue effects, real-time monitoring, software verification, biocompatibility and human factors. FDA also confirmed that devices in this category require 510(k) submissions. For developers, it is a clear example of how a pioneering De Novo classification can establish a regulatory pathway for subsequent technologies.

2. FDA Establishes Class II Regulatory Category for STI Molecular Tests Using Home-Collected Samples

Publication/effective date: October 9, 2026

FDA codified 21 CFR 866.3385 for IVD systems that detect nucleic acids from nonviral sexually transmitted pathogens in specimens collected by users at home but tested by a clinical laboratory. The Class II category carries special controls. It follows the November 2023 De Novo classification of LetsGetChecked's Simple 2 Test rather than representing a newly cleared product.

This is not an at-home, immediate-result test. The regulatory controls cover collection devices, specimen handling, analytical and clinical performance, false-result risks, usability and labeling. FDA specifies a 510(k) pathway for devices in the category, clarifying how decentralized specimen collection can connect with regulated laboratory testing.

3. MRI2CT Wins FDA 510(k) Clearance for NextMR, Generating 3D Bone Views from Standard Spine MRI

Announced: October 7, 2026

MRI2CT announced FDA 510(k) clearance for NextMR, an AI imaging platform that reconstructs 3D lumbar-spine and sacral bone morphology directly from conventional 1.5T or 3T MRI scans. The company reports generating its output in under 60 seconds, with views available through a web interface or compatible DICOM environments.

According to the company, retrospective testing in 145 adults with paired MRI and CT studies demonstrated submillimeter cortical-bone delineation accuracy. The product is not authorized as a universal replacement for CT, and compatibility with third-party surgical systems must be assessed separately. The technology nonetheless points toward more integrated diagnostic imaging and surgical-planning workflows.

4. Smith+Nephew Commercially Launches CARTIHEAL AGILI-C Cartilage Repair Implant in Europe

Announced: October 6, 2026

Smith+Nephew announced European commercialization of CARTIHEAL AGILI-C, an aragonite-based biphasic scaffold designed to repair cartilage and underlying bone in the knee. The implant has already been available in the United States since 2024. Initial European markets include Germany, France, Italy and the United Kingdom.

The company cites five-year follow-up from a multicenter randomized study showing favorable Knee injury and Osteoarthritis Outcome Scores (KOOS) compared with microfracture or debridement. Its European rollout illustrates how clinical evidence generated for an innovative implant can support expansion across regions while addressing local market-access and procedural-adoption requirements.

5. XCath Iris Remote Stroke-Thrombectomy Robot Selected for FDA TAP Pilot

Announced: October 8, 2026

XCath Robotics announced that FDA accepted the Iris Surgical Robotic System into its Total Product Life Cycle Advisory Program (TAP) pilot. Iris is under development for remote robot-assisted mechanical thrombectomy in acute ischemic stroke and received Breakthrough Device Designation in September 2026.

TAP provides structured engagement with FDA advisers and facilitates discussions spanning clinical evidence, regulation, reimbursement and adoption. XCath also reported a remote robotic thrombectomy demonstration in Panama in March 2026. TAP participation is not clearance or approval: Iris remains investigational. The program is noteworthy for its combined regulatory and market-access approach to a device intended to address geographic gaps in specialist stroke care.

6. Zenflow Raises $52 Million and Signs Exclusive U.S. Distribution Agreement with Cook Medical

Announced: October 5, 2026

Zenflow closed an oversubscribed $52 million Series D round led by Cook Medical and signed an exclusive U.S. distribution agreement with Cook. The transaction will fund commercialization of the Zenflow Spring System, which received FDA PMA approval in December 2025. This week's milestone concerns financing and distribution, not a new FDA approval.

The Spring implant provides an office-based minimally invasive option for benign prostatic hyperplasia (BPH), with multiple sizes and intended reversibility if clinically needed. The arrangement combines Zenflow's device-development platform with Cook's urology distribution infrastructure—an instructive partnership model for companies moving from pivotal development into commercial launch.

7. Japan's HOYA Agrees to Sell Ophthalmic Surgical-Instrument Maker MID Labs to Bausch + Lomb

Announced: October 9, 2026

HOYA signed a definitive agreement to sell 100% of Medical Instrument Development Laboratories (MID Labs) to Bausch + Lomb. The U.S.-based subsidiary manufactures and sells vitreoretinal surgical instruments and reported $34 million in revenue for the fiscal year ended March 2026. The purchase price was not disclosed; the share transfer is scheduled for November 2, 2026.

HOYA cited portfolio optimization and capital allocation, expecting no material impact on current-year consolidated results. The transaction puts specialized ophthalmic-instrument manufacturing under an established eye-care manufacturer and highlights how Japanese MedTech groups continue to reshape international business and supply-chain portfolios. Signing should not be confused with the closing date.
